You can print out the form through the link below and bring it in on your first visit, or arrive early to your appointment to fill it out in office. The paperwork provides us with general information about yourself and your condition. The Massage Therapist will review your paperwork, ask any necessary follow-up questions, and discuss a care plan with you.
